If you’ve been a client of Community Action in the past 12 months please take this survey.
Iowa’s Community Action Network is conducting a study of the needs individuals and families may be experiencing in their lives. Information for the study will be collected by surveying our customers.
The results will be considered by for planning, developing, and delivering agency programs and services to the people that reside in our neighborhoods and communities.
The survey takes approximately 15 minutes to complete; there are 50+ multiple choice type questions and a few questions that are open-ended (questions you can type a response). All replies are anonymous and will be treated confidentially.
